HID: lg: fix ReportDescriptor for Logitech Formula Vibration

By default the Logitech Formula Vibration presents a combined accel/brake
axis ('Y'). This patch modifies the HID descriptor to present seperate
accel/brake axes ('Y' and 'Z').
