V4L/DVB (5109): Convert cx88-blackbird to use video_ioctl2

This patch finishes cx88-blackbird conversion to use video_ioctl2. Video
standards are generated automatically inside videodev.c. the big ioctl
parser is removed, using, instead, video_ioctl2.
