If the function platform_get_irq() failed, the negative value
returned will not be detected here. So fix error handling in
exynos_ohci_probe(). And when get irq failed, the function
platform_get_irq() logs an error message, so remove redundant
message here.

Add the code for getting generic PHYs from standard device tree array
from the main controller device node. This is a first step in resolving
the conflict between Exynos EHCI/OHCI sub-nodes and generic USB device
bindings. Later the sub-nodes currently used for assigning PHYs to root
ports of the controller will be removed making a place for the generic
USB device bindings nodes.

Commit 69bec72598 ("USB: core: let USB device know device node") added
support for attaching devicetree node for USB devices. Those nodes are
children of their USB host controller. However Exynos EHCI and OHCI
driver bindings already define child-nodes for each physical root hub
port and assigns respective PHY controller and parameters to them. Those
bindings predates support for USB device tree nodes.
To mitigate the side-effects of the conflict between those bindings,
lets reset Exynos host controller of_node pointer before registering it
to USB subsystem. This fixes the issue raised by the commit 01fdf179f4
("usb: core: skip interfaces disabled in devicetree"), which incorrectly
disabled some devices on Exynos based boards.

Now that the SPDX tag is in all USB files, that identifies the license
in a specific and legally-defined manner. So the extra GPL text wording
can be removed as it is no longer needed at all.
This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.
No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.
Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

Returning from for_each_available_child_of_node() loop requires cleaning
up node refcount. Error paths lacked it so for example in case of
deferred probe, the refcount of phy node was left increased.

On enabling CONFIG_OF_SELFTEST which enables CONFIG_OF_DYNAMIC,
we found out that while getting PHYs for the controller we were
doing an extra of_node_put on the child node in our routines -
exynos_e/ohci_get_phy().
This child is however already put by of_get_next_available_child()
which does a of_node_put() on the "prev" node. So there's no point
in putting the same node again in our routine.

Now that we have completely moved from older USB-PHY drivers
to newer GENERIC-PHY drivers for PHYs available with USB controllers
on Exynos series of SoCs, we can remove the support for the same
in our host drivers too.
We also defer the probe for our host in case we end up getting
EPROBE_DEFER error when getting PHYs.

Since we want to keep support for both older usb-phys as well as the
newer generic phys, lets first get the generic PHYs and fallback to
older USB-PHYs only when we fail to get the former.
This should fix the issue with ehci-exynos and ohci-exynos, wherein
in the absence of SAMSUNG_USB2PHY config symbol, we end up getting
the NOP_USB_XCEIV phy when the same is enabled. And thus the PHYs
are not configured properly.

Using devm_ioremap_resource() API should actually be preferred over
devm_ioremap(), since the former request the mem region first and then
gives back the ioremap'ed memory pointer.
devm_ioremap_resource() calls request_mem_region(), therby preventing
other drivers to make any overlapping call to the same region.

Add support to consume phy provided by Generic phy framework.
Keeping the support for older usb-phy intact right now, in order
to prevent any functionality break in absence of relevant
device tree side change for ohci-exynos.
Once we move to new phy in the device nodes for ohci, we can
remove the support for older phys.

Individual controller driver has different requirement for wakeup
setting, so move it from core to itself. In order to align with
current etting the default wakeup setting is enabled (except for
chipidea host).

Pull DMA mask updates from Russell King:
"This series cleans up the handling of DMA masks in a lot of drivers,
fixing some bugs as we go.
Some of the more serious errors include:
- drivers which only set their coherent DMA mask if the attempt to
set the streaming mask fails.
- drivers which test for a NULL dma mask pointer, and then set the
dma mask pointer to a location in their module .data section -
which will cause problems if the module is reloaded.
To counter these, I have introduced two helper functions:
- dma_set_mask_and_coherent() takes care of setting both the
streaming and coherent masks at the same time, with the correct
error handling as specified by the API.
- dma_coerce_mask_and_coherent() which resolves the problem of
drivers forcefully setting DMA masks. This is more a marker for
future work to further clean these locations up - the code which
creates the devices really should be initialising these, but to fix
that in one go along with this change could potentially be very
disruptive.
The last thing this series does is prise away some of Linux's addition
to "DMA addresses are physical addresses and RAM always starts at
zero". We have ARM LPAE systems where all system memory is above 4GB
physical, hence having DMA masks interpreted by (eg) the block layers
as describing physical addresses in the range 0..DMAMASK fails on
these platforms. Santosh Shilimkar addresses this in this series; the
patches were copied to the appropriate people multiple times but were
ignored.
Fixing this also gets rid of some ARM weirdness in the setup of the
max*pfn variables, and brings ARM into line with every other Linux
architecture as far as those go"

The correct way for a driver to specify the coherent DMA mask is
not to directly access the field in the struct device, but to use
dma_set_coherent_mask(). Only arch and bus code should access this
member directly.
Convert all direct write accesses to using the correct API.

The non-DT for EXYNOS SoCs is not supported from v3.11.
Thus, there is no need to support non-DT for Exynos OHCI driver.
The 'include/linux/platform_data/usb-ohci-exynos.h' file has been
used for non-DT support. Thus, the 'usb-ohci-exynos.h' file can
be removed.

Many USB host drivers contain code such as:
if (!pdev->dev.dma_mask)
pdev->dev.dma_mask = &tegra_ehci_dma_mask;
... where tegra_ehci_dma_mask is a global. I suspect this code originated
in commit 4a53f4e "USB: ehci-tegra: add probing through device tree" and
was simply copied everywhere else.
This works fine when the code is built-in, but can cause a crash when the
code is in a module. The first module load sets up the dma_mask pointer,
but if the module is removed and re-inserted, the value is now non-NULL,
and hence is not updated to point at the new location, and hence points
at a stale location within the previous module load address, which in
turn causes a crash if the pointer is de-referenced.
The simplest way of solving this seems to be to copy the code from
ehci-platform.c, which uses the coherent_dma_mask as the target for the
dma_mask pointer.

The definitions have moved to include/linux/usb/samsung-usb-phy.h,
and plat/usb-phy.h is unavailable from drivers in a multiplatform
configuration.
Also fix up the plat/usb-phy.h header file to use the definitions
from the new header instead of providing a separate copy.

Exynos5440 does not require any explict USB phy configuration. So skip
the USB phy configuration for Exynos5440 based platforms.

This patch changes the name of USB ohci header from 'usb-exynos.h'
to 'usb-ohci-exynos.h'. This is because this header file has
the platdata for only EXYNOS OHCI.

Two new PHY drivers coming here: one for Samsung,
one for OMAP. Both architectures are adding USB3
support to mainline kernel.
The PHY layer now allows us to have mulitple PHYs
of the same type, which is necessary for platforms
which provide more than one USB peripheral port.
There's also a few cleanups here: removal of __dev*
annotations, conversion of a cast to to_delayed_work(),
and mxs-phy learns about ->set_suspend.

In the former code, we have a race condition between the first interrupt
and the regs field initilization in the usb_hcd structure.
If the OHCI irq fires before hcd->regs is set, we are getting a null
pointer dereference in ohci_irq.
When calling usb_add_hcd(), it first executes the reset() callback,
then enables the ohci interrupt, and finally executes the start()
callback. So moving the ohci_init() call which actually initializes the
reg field from start() to reset() should remove the race.
Tested by enabling the external HSIC hub in the bootloader on an exynos5
machine and booting. With the former code, this triggers an early interrupt
about 50% of the boots and a subsequent kernel panic in ohci_irq when trying
to access the registers.

Merge ohci_finish_controller_resume with ohci_resume as suggested by Alan
Stern. Since ohci_finish_controller_resume no longer exists, update the
various OHCI drivers to call ohci_resume() instead. Some drivers used to set
themselves the bit HCD_FLAG_HW_ACCESSIBLE, which is now handled by
ohci_resume().
