James Smart e26422fe91 lpfc: The lpfc driver does not issue RFF_ID and RFT_ID in the correct sequence
The order (it's a shall, but hard to dictate after the fact) is given in
FC-SCM - kind of.  SCM indicates what shall be implemented, lists it as (a),
(b), (c), but actually doesn't say it has to be in that order.  The only hard
requirement, called out in FCP-4, is that you must register your FC-4 Type
(via RFT_ID) before registering FC-4 Type Features (via RFF_ID), which makes
sense. We obviously violated this and there were some switches (or newer fw in
them) that enforced it.  The other rule of thumbs are: register your data with
the switch first, then register for SCRs, then do queries about the fabric,
with the SCRs telling you of changes post the queries.

Alexey Kardashevskiy 8b2564ec74 lpfc: Fix breakage on big endian kernels
This reverts 4fbdf9cb it breaks LPFC on POWER7 machine, big endian kernel.
Without this, the kernel enters an infinite oops loop.

James Smart be6bb94100 lpfc: Fix premature release of rpi bit in bitmask
Currently, the driver plays off the fact that older sli4 adapters have a
different rpi access pattern that allowed for the rpi reference to be
released earlier in the teardown sequence, allowing the driver to recycle
the rpi value sooner. Newer sli4 adapters have a different access pattern that
requires us to wait for a later mailbox completion. This changes the put
call location on the newer sli4 adapters.

Symptoms of the error are "0110 ELS" and the "0372 iotag" errors.

James Smart ea4142f6b1 lpfc: Fix host reset escalation killing all IOs.
Fix host reset escalation killing all IOs.

SLI-3 adapters will use a new host template. The template differs
from SLI-4 adapters in that it does not have an eh_host_reset_handler.

Lpfc has traditionally never had a host_reset. The host reset
handler was added when we ran into a stuck hardware condition on a
SLI-4 adapter. The host_reset will reset and reinit the pci function,
clearing the hardware condition.

Unfortunately, the host reset handler uses attach/detach code paths,
which makes scsi_add_host() and scsi_remove_host() calls. Meaning, a
host_reset will completely remove the scsi_host from the system. As a
new call to scsi_add_host() is made, the shost# changes, which results
in completely new scsi_devices and device names. All the older scsi
devices on the old shost# are now orphaned and unrecoverable.

We realize we need to re-implement the host_reset_handler so the scsi_host
stays registered across the host_reset, but that will be a rather
lengthy effort. In the short term, we had an immediate need to restore
the SLI-3 devices to their working behavior, with the easiest path being
to remove their host_reset handler.

James Smart d454c91f74 lpfc: correct device removal deadlock after link bounce
This patch, applicable to 8G/4G/2G adapters, adds a call that
resumes transmit operations after a link bounce. Without it, targets
that tried to suspend exchanges after a link bounce (such as tape devices
using sequence level error recovery) would never resume io operation,
causing scan failures, and eventually deadlocks if a device removal
request is made.

Christoph Hellwig efc3c1df5f scsi: remove ->change_queue_type method
Since we got rid of ordered tag support in 2010 the prime use case of
switching on and off ordered tags has been obsolete.  The other function
of enabling/disabling tagging entirely has only been correctly implemented
by the 53c700 driver and isn't generally useful.

Christoph Hellwig c40ecc12cf scsi: avoid ->change_queue_depth indirection for queue full tracking
All drivers use the implementation for ramping the queue up and down, so
instead of overloading the change_queue_depth method call the
implementation diretly if the driver opts into it by setting the
track_queue_depth flag in the host template.

Note that a few drivers validated the new queue depth in their
change_queue_depth method, but as we never go over the queue depth
set during slave_configure or the sysfs file this isn't nessecary
and can safely be removed.

Christoph Hellwig c8b09f6fb6 scsi: don't set tagging state from scsi_adjust_queue_depth
Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth.  For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.

Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway.  The one other case looks
broken, but I've kept the behavior as-is for now.

Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.

Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Christoph Hellwig 2ecb204d07 scsi: always assign block layer tags if enabled
Allow a driver to ask for block layer tags by setting .use_blk_tags in the
host template, in which case it will always see a valid value in
request->tag, similar to the behavior when using blk-mq.  This means even
SCSI "untagged" commands will now have a tag, which is especially useful
when using a host-wide tag map.

Christoph Hellwig 5066863337 scsi: remove abuses of scsi_populate_tag
Unless we want to build a SPI tag message we should just check SCMD_TAGGED
instead of reverse engineering a tag type through the use of
scsi_populate_tag_msg.

Also rename the function to spi_populate_tag_msg, make it behave like the
other spi message helpers, and move it to the spi transport class.

Alexander Gordeev 4f871e1b27 lpfc: Use pci_enable_msix_range() instead of pci_enable_msix()
As result of deprecation of MSI-X/MSI enablement functions
pci_enable_msix() and pci_enable_msi_block() all drivers
using these two interfaces need to be updated to use the
new pci_enable_msi_range()  or pci_enable_msi_exact()
and pci_enable_msix_range() or pci_enable_msix_exact()
interfaces.

Daniel Borkmann a7901acc4a lpfc: do not feed jiffies as random seed from lpfc driver
In prandom we have already reseeding mechanisms that trigger
periodically from a much better entropy source than just
feeding in jiffies through lpfc_mbx_cmpl_fcf_scan_read_fcf_rec()
[what a function name 8-)]. Therefore, just remove this.

James Smart 27f344eb15 lpfc: Add iotag memory barrier
Add a memory barrier to ensure the valid bit is read before
any of the cqe payload is read. This fixes an issue seen
on Power where the cqe payload was getting loaded before
the valid bit. When this occurred, we saw an iotag out of
range error when a command completed, but since the iotag
looked invalid the command didn't get completed to scsi core.
Later we hit the command timeout, attempted to abort the command,
then waited for the aborted command to get returned. Since the
adapter already returned the command, we timeout waiting,
and end up escalating EEH all the way to host reset. This
patch fixes this issue.

On lpfc_debugfs_initialize function the dumpHostSlim member setup happens

This patch implements the changes requested by Jeremy Linton:

The patch revises the command issuing behavior, detecting cases where the
Task Mgmt command may have completed but with a non-successful status, which it
previously treated as a successful TMF. The patch also corrects a flushing of
I/O that was done which should only be done on successful TMF completion.

Jon Mason 453193e042 [SCSI] lpfc: remove unnecessary read of PCI_CAP_ID_EXP
The PCIE capability offset is saved during PCI bus walking.  It will
remove an unnecessary search in the PCI configuration space if this
value is referenced instead of reacquiring it.  Also, pci_is_pcie is a
better way of determining if the device is PCIE or not (as it uses the
same saved PCIE capability offset).

Jingoo Han 08b7e10716 SCSI: remove unnecessary pci_set_drvdata()
Since commit 0998d06310
(device-core: Ensure drvdata = NULL when no driver is bound),
the driver core clears the driver data to NULL after device_release
or on probe failure. Thus, it is not needed to manually clear the

The patch set is mostly driver updates (usf, zfcp, lpfc, mpt2sas,
 megaraid_sas, bfa, ipr) and a few bug fixes.  Also of note is that the
 Buslogic driver has been rewritten to a better coding style and 64 bit support
 added.  We also removed the libsas limitation on 16 bytes for the command size
 (currently no drivers make use of this).
