Christophe Leroy 3160e84aba powerpc/mm: Fix CONFIG_PPC_KUAP_DEBUG on PPC32
commit feb8e960d7 upstream.

CONFIG_PPC_KUAP_DEBUG is not selectable because it depends on PPC_32
which doesn't exists.

Fixing it leads to a deadlock due to a vital register getting
clobbered in _switch().

Change dependency to PPC32 and use r0 instead of r4 in _switch()

Michael Ellerman b48331b52a powerpc/kuap: PPC_KUAP_DEBUG should depend on PPC_KUAP
commit 61da50b76b upstream.

Currently you can enable PPC_KUAP_DEBUG when PPC_KUAP is disabled,
even though the former has not effect without the latter.

Fix it so that PPC_KUAP_DEBUG can only be enabled when PPC_KUAP is
enabled, not when the platform could support KUAP (PPC_HAVE_KUAP).

Christoph Hellwig f2902a2fb4 powerpc: use the generic dma coherent remap allocator
This switches to using common code for the DMA allocations, including
potential use of the CMA allocator if configured.

Switching to the generic code enables DMA allocations from atomic
context, which is required by the DMA API documentation, and also
adds various other minor features drivers start relying upon.  It
also makes sure we have on tested code base for all architectures
that require uncached pte bits for coherent DMA allocations.

Another advantage is that consistent memory allocations now share
the general vmalloc pool instead of needing an explicit careout
from it.

Christophe Leroy f7a0bf7d90 powerpc/32s: add an option to exclusively select powerpc 601
Powerpc 601 is rather old powerpc which as some important
limitations compared to other book3s/32 powerpcs:
- No Timebase.
- Common BATs for instruction and data.
- No execution protection in segment registers.
- No RI bit in MSR
- ...

It is starting to be difficult and cumbersome to maintain
kernels that are compatible both with 601 and other 6xx cores.

Create a compiletime option to exclusively select either powerpc 601
or other 6xx.

Alexandre Ghiti 4eb0716e86 hugetlb: allow to free gigantic pages regardless of the configuration
On systems without CONTIG_ALLOC activated but that support gigantic pages,
boottime reserved gigantic pages can not be freed at all.  This patch
simply enables the possibility to hand back those pages to memory
allocator.

  - xmon, our crash handler, gets support for a read only mode where operations
    that could change memory or otherwise disturb the system are disabled.

Christophe Leroy a68c31fc01 powerpc/32s: Implement Kernel Userspace Access Protection
This patch implements Kernel Userspace Access Protection for
book3s/32.

Due to limitations of the processor page protection capabilities,
the protection is only against writing. read protection cannot be
achieved using page protection.

The previous patch modifies the page protection so that RW user
pages are RW for Key 0 and RO for Key 1, and it sets Key 0 for
both user and kernel.

This patch changes userspace segment registers are set to Ku 0
and Ks 1. When kernel needs to write to RW pages, the associated
segment register is then changed to Ks 0 in order to allow write
access to the kernel.

In order to avoid having the read all segment registers when
locking/unlocking the access, some data is kept in the thread_struct
and saved on stack on exceptions. The field identifies both the
first unlocked segment and the first segment following the last
unlocked one. When no segment is unlocked, it contains value 0.

As the hash_page() function is not able to easily determine if a
protfault is due to a bad kernel access to userspace, protfaults
need to be handled by handle_page_fault when KUAP is set.

Christophe Leroy 31ed2b13c4 powerpc/32s: Implement Kernel Userspace Execution Prevention.
To implement Kernel Userspace Execution Prevention, this patch
sets NX bit on all user segments on kernel entry and clears NX bit
on all user segments on kernel exit.

Note that powerpc 601 doesn't have the NX bit, so KUEP will not
work on it. A warning is displayed at startup.

Christophe Leroy 2679f9bd0a powerpc/8xx: Add Kernel Userspace Access Protection
This patch adds Kernel Userspace Access Protection on the 8xx.

When a page is RO or RW, it is set RO or RW for Key 0 and NA
for Key 1.

Up to now, the User group is defined with Key 0 for both User and
Supervisor.

By changing the group to Key 0 for User and Key 1 for Supervisor,
this patch prevents the Kernel from being able to access user data.

At exception entry, the kernel saves SPRN_MD_AP in the regs struct,
and reapply the protection. At exception exit it restores SPRN_MD_AP
with the value saved on exception entry.

the KUAP state, and modifies setup_32 to call them on exceptions
from kernel.

The macros are defined as empty by default for when CONFIG_PPC_KUAP

Michael Ellerman 890274c2dc powerpc/64s: Implement KUAP for Radix MMU
Kernel Userspace Access Prevention utilises a feature of the Radix MMU
which disallows read and write access to userspace addresses. By
utilising this, the kernel is prevented from accessing user data from
outside of trusted paths that perform proper safety checks, such as
copy_{to/from}_user() and friends.

Userspace access is disabled from early boot and is only enabled when
performing an operation like copy_{to/from}_user(). The register that
controls this (AMR) does not prevent userspace from accessing itself,
so there is no need to save and restore when entering and exiting
userspace.

When entering the kernel from the kernel we save AMR and if it is not
blocking user access (because eg. we faulted doing a user access) we
reblock user access for the duration of the exception (ie. the page
fault) and then restore the AMR when returning back to the kernel.

This feature can be tested by using the lkdtm driver (CONFIG_LKDTM=y)
and performing the following:

  # (echo ACCESS_USERSPACE) > [debugfs]/provoke-crash/DIRECT

If enabled, this should send SIGSEGV to the thread.

We also add paranoid checking of AMR in switch and syscall return
under CONFIG_PPC_KUAP_DEBUG.

Christophe Leroy de78a9c42a powerpc: Add a framework for Kernel Userspace Access Protection
This patch implements a framework for Kernel Userspace Access
Protection.

Then subarches will have the possibility to provide their own
implementation by providing setup_kuap() and
allow/prevent_user_access().

Some platforms will need to know the area accessed and whether it is
accessed from read, write or both. Therefore source, destination and
size and handed over to the two functions.

mpe: Rename to allow/prevent rather than unlock/lock, and add
read/write wrappers. Drop the 32-bit code for now until we have an
implementation for it. Add kuap to pt_regs for 64-bit as well as
32-bit. Don't split strings, use pr_crit_ratelimited().

Michael Ellerman 8adddf349f powerpc/mm/radix: Make Radix require HUGETLB_PAGE
Joel reported weird crashes using skiroot_defconfig, in his case we
jumped into an NX page:

  kernel tried to execute exec-protected page (c000000002bff4f0) - exploit attempt? (uid: 0)
  BUG: Unable to handle kernel instruction fetch
  Faulting instruction address: 0xc000000002bff4f0

Looking at the disassembly, we had simply branched to that address:

  c000000000c001bc  49fff335    bl     c000000002bff4f0

But that didn't match the original kernel image:

When STRICT_KERNEL_RWX is enabled, and we're using the radix MMU, we
call radix__change_memory_range() late in boot to change page
protections. We do that both to mark rodata read only and also to mark
init text no-execute. That involves walking the kernel page tables,
and clearing _PAGE_WRITE or _PAGE_EXEC respectively.

With radix we may use hugepages for the linear mapping, so the code in
radix__change_memory_range() uses eg. pmd_huge() to test if it has
found a huge mapping, and if so it stops the page table walk and
changes the PMD permissions.

However if the kernel is built without HUGETLBFS support, pmd_huge()
is just a #define that always returns 0. That causes the code in
radix__change_memory_range() to incorrectly interpret the PMD value as
a pointer to a PTE page rather than as a PTE at the PMD level.

We can see this using `dv` in xmon which also uses pmd_huge():

The end result is we treat the value at 0xc000000000000100 as a PTE
and clear _PAGE_WRITE or _PAGE_EXEC, potentially corrupting the code
at that address.

In Joel's specific case we cleared the sign bit in the offset of the
branch, causing a backward branch to turn into a forward branch which
caused us to branch into a non-executable page. However the exact
nature of the crash depends on kernel version, compiler version, and
other factors.

We need to fix radix__change_memory_range() to not use accessors that
depend on HUGETLBFS, but we also have radix memory hotplug code that
uses pmd_huge() etc that will also need fixing. So for now just
disallow the broken combination of Radix with HUGETLBFS disabled.

The only defconfig we have that is affected is skiroot_defconfig, so
turn on HUGETLBFS there so that it still gets Radix.

setting so there is no need to write it explicitly.

Also since commit f467c5640c ("kconfig: only write '# CONFIG_FOO

regardless of 'default n' being present or not:

    One side effect of (and the main motivation for) this change is making
    the following two definitions behave exactly the same:

    With this change, neither of these will generate a

Christophe Leroy aa0ab02ba9 powerpc/mm/slice: Fix hugepage allocation at hint address on 8xx
On the 8xx, the page size is set in the PMD entry and applies to
all pages of the page table pointed by the said PMD entry.

When an app has some regular pages allocated (e.g. see below) and tries
to mmap() a huge page at a hint address covered by the same PMD entry,
the kernel accepts the hint allthough the 8xx cannot handle different
page sizes in the same PMD entry.

In order to fix this, this patch uses the address space "slices"
implemented for BOOK3S/64 and enhanced to support PPC32 by the
preceding patch.

This patch modifies the context.id on the 8xx to be in the range
[1:16] instead of [0:15] in order to identify context.id == 0 as
not initialised contexts as done on BOOK3S

This patch activates CONFIG_PPC_MM_SLICES when CONFIG_HUGETLB_PAGE is
selected for the 8xx

Alltough we could in theory have as many slices as PMD entries, the
current slices implementation limits the number of low slices to 16.
This limitation is not preventing us to fix the initial issue allthough
it is suboptimal. It will be cured in a subsequent patch.

Christophe Leroy cd99ddbea2 powerpc/8xx: Only perform perf counting when perf is in use.
In TLB miss handlers, updating the perf counter is only useful
when performing a perf analysis. As it has a noticeable overhead,
let's only do it when needed.

In order to do so, the exit of the miss handlers will be patched
when starting/stopping 'perf': the first register restore
instruction of each exit point will be replaced by a jump to
the counting code.

Once this is done, CONFIG_PPC_8xx_PERF_EVENT becomes useless as
this feature doesn't add any overhead.

Michael Ellerman 1fd6c02207 powerpc/mm: Add a CONFIG option to choose if radix is used by default
Currently if the hardware supports the radix MMU we will use
it, *unless* "disable_radix" is passed on the kernel command line.

However some users would like the reverse semantics. ie. The kernel
uses the hash MMU by default, unless radix is explicitly requested on
the command line.

So add a CONFIG option to choose whether we use radix by default or
not, and expand the disable_radix command line option to allow
"disable_radix=no" which *enables* radix.

Michael Ellerman 4e00374704 powerpc/64s: Replace CONFIG_PPC_STD_MMU_64 with CONFIG_PPC_BOOK3S_64
CONFIG_PPC_STD_MMU_64 indicates support for the "standard" powerpc MMU
on 64-bit CPUs. The "standard" MMU refers to the hash page table MMU
found in "server" processors, from IBM mainly.

Currently CONFIG_PPC_STD_MMU_64 is == CONFIG_PPC_BOOK3S_64. While it's
annoying to have two symbols that always have the same value, it's not
quite annoying enough to bother removing one.

However with the arrival of Power9, we now have the situation where
CONFIG_PPC_STD_MMU_64 is enabled, but the kernel is running using the
Radix MMU - *not* the "standard" MMU. So it is now actively confusing
to use it, because it implies that code is disabled or inactive when
the Radix MMU is in use, however that is not necessarily true.

So s/CONFIG_PPC_STD_MMU_64/CONFIG_PPC_BOOK3S_64/, and do some minor
formatting updates of some of the affected lines.

This will be a pain for backports, but c'est la vie.

Greg Kroah-Hartman b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

Christophe Leroy 04f61b660f powerpc/8xx: remove CONFIG_8xx
Two config options exist to define powerpc MPC8xx:
* CONFIG_PPC_8xx
* CONFIG_8xx

arch/powerpc/platforms/Kconfig.cputype has contained the following
comment about CONFIG_8xx item for some years:
"# this is temp to handle compat with arch=ppc"

There is no more users of CONFIG_8xx, so remove it.
