Backend drivers shouldn't transistion to CLOSED unless the frontend is
CLOSED. If a backend does transition to CLOSED too soon then the
frontend may not see the CLOSING state and will not properly shutdown.
So, treat an unexpected backend CLOSED state the same as CLOSING.

"This is workqueue updates for v3.7-rc1. A lot of activities this
round including considerable API and behavior cleanups.
* delayed_work combines a timer and a work item. The handling of the
timer part has always been a bit clunky leading to confusing
cancelation API with weird corner-case behaviors. delayed_work is
updated to use new IRQ safe timer and cancelation now works as
expected.
* Another deficiency of delayed_work was lack of the counterpart of
mod_timer() which led to cancel+queue combinations or open-coded
timer+work usages. mod_delayed_work[_on]() are added.
These two delayed_work changes make delayed_work provide interface
and behave like timer which is executed with process context.
* A work item could be executed concurrently on multiple CPUs, which
is rather unintuitive and made flush_work() behavior confusing and
half-broken under certain circumstances. This problem doesn't
exist for non-reentrant workqueues. While non-reentrancy check
isn't free, the overhead is incurred only when a work item bounces
across different CPUs and even in simulated pathological scenario
the overhead isn't too high.
All workqueues are made non-reentrant. This removes the
distinction between flush_[delayed_]work() and
flush_[delayed_]_work_sync(). The former is now as strong as the
latter and the specified work item is guaranteed to have finished
execution of any previous queueing on return.
* In addition to the various bug fixes, Lai redid and simplified CPU
hotplug handling significantly.
* Joonsoo introduced system_highpri_wq and used it during CPU
hotplug.
There are two merge commits - one to pull in IRQ safe timer from
tip/timers/core and the other to pull in CPU hotplug fixes from
wq/for-3.6-fixes as Lai's hotplug restructuring depended on them."
Fixed a number of trivial conflicts, but the more interesting conflicts
were silent ones where the deprecated interfaces had been used by new
code in the merge window, and thus didn't cause any real data conflicts.

The pseries hvterm driver only registers a udbg backend (for xmon and
other low level debugging mechanisms) when hvc0 is recognized as the
firmware console at boot time, not if it's detected later on, for
example because the firmware is using a graphics card.
This can make debugging challenging especially under X11, and there's
really no good reason for that limitation, so let's hookup udbg
whenever hvc0 is detected instead.

hvc_console has two methods to instanciate the consoles.
hvc_instanciate is meant to be called at early boot, while hvc_alloc is
called for more dynamically probed objects.
Currently, it only deals with adding kernel consoles in the former case,
which means for example that if a console only uses dynamic probing, it
will never be usable as a kernel console even when specifying
console=hvc0 explicitly, which could be considered annoying...
More specifically, on pseries, we only do the early instanciate for the
console currently used by the firmware, so if you have your firmware
configured to go to a video card, for example, you cannot get your
kernel console, oops messages, etc... on your serial port or hypervisor
console, which would be handy to deal with oopses.
This fixes it by checking if hvc_console.flags & CON_ENABLED is set when
registering a new dynamic console, and if not, redo the index check and
re-register the console if the index matches, allowing console=hvcN to
work.

flush[_delayed]_work_sync() are now spurious. Mark them deprecated
and convert all users to flush[_delayed]_work().
If you're cc'd and wondering what's going on: Now all workqueues are
non-reentrant and the regular flushes guarantee that the work item is
not pending or running on any CPU on return, so there's no reason to
use the sync flushes at all and they're going away.
This patch doesn't make any functional difference.

tty_port_register_driver but still have tty_port available before
tty_register_driver we use newly added tty_port_link_device.
The rest of the drivers that still do not provide tty_struct <->

We need to make sure that those parameters are setup to be correct.
As such the value of 0 is deemed invalid and we find that we
bail out. The hypervisor sets by default all of them to be zero
and when the hypercall is done does a simple:
a.value = d->arch.hvm_domain.params[a.index];
Which means that if the Xen toolstack forgot to setup the proper
HVM_PARAM_CONSOLE_EVTCHN (or the PFN one), we would get the
default value of 0 and use that.

If kzalloc() returns a NULL here, we pass a NULL to
xencons_disconnect_backend() which will cause an Oops.
Also I removed the __GFP_ZERO while I was at it since kzalloc() implies
__GFP_ZERO.

A -next commit "TTY: HVC, use tty from tty_port" switched the driver
to use tty_port helper for tty refcounting. But it omitted to remove

- use tty, not hp->tty wherever possible
- pass tty down to some functions and go to step one
- do not defer tty_hangup calls -- it is as simple as schedule_work,
so might be called with hp->lock held
- do not defer tty buffer flips -- since the driver does not use
low_latency (it cannot actually), the flip is a simple tail move
plus schedule_work. It will make our life easier in the next patch.

It is in termios cflags. So change the test in hvsi_recv_control to do
the right thing. Previously it was actually testing TTY_LDISC_OPEN
bit, i.e. whether an ldisc is active. And yes, it is most of the time.

No refcounting, just a switch. The locking in the driver prevents
races, so in fact the refcounting is not needed. But while we have a
tty in tty_port, don't duplicate that and remove the one from
hvcs_struct.

n_outbuf is left unprotected in hvc_hangup now, because there is no
point to hold any lock, since other uses are unprotected too.

This patch implements support for multiple consoles:
consoles other than the first one are setup using the traditional xenbus
and grant-table based mechanism.
We use a list to keep track of the allocated consoles, we don't
expect too many of them anyway.

Checking if tty->index is in bounds is not needed. The tty has the
index set in the initial open. This is done in get_tty_driver. And it
can be only in interval <0,driver->num).
So remove the tests which check exactly this interval. Some are
left untouched as they check against the current backing device count.
(Leaving apart that the check is racy in most of the cases.)

All num, magic and owner are set by alloc_tty_driver. No need to
re-set them on each allocation site.
pti driver sets something different to what it passes to
alloc_tty_driver. It is not a bug, since we don't use the lines
parameter in any way. Anyway this is fixed, and now we do the right
thing.

We transform the offenders into a test of irq <= 0 which will be ok while
the ARM people get their platform sorted. Once that is done (or in a while
if they don't do it anyway) then we will change them all to !irq checks.
For arch specific drivers that are already using NO_IRQ = 0 we just test
against zero so we don't need to re-review them later.
