This is a bit hack, but returning error when driver is not tuned yet
causes DVBv5 zap stop polling DVBv5 statistics. Thus return 0 even
callback is called during invalid device state.

There should be clearly some lock in order to make sure firmware
command in execution is not disturbed by another command. It has
worked as callbacks are serialized somehow pretty well and command
execution happens usually without any delays.

The DVB API was originally defined using typedefs. This is against
Kernel CodingStyle, and there's no good usage here. While we can't
remove its usage on userspace, we can avoid its usage in Kernelspace.
So, let's do it.

Only supported modulation for DVB-S is QPSK. Modulation parameter
contains invalid value for DVB-S on some cases, which leads driver
refusing tuning attempt. Due to that, hard code modulation to QPSK
in case of DVB-S.

Tuner PLL lock flag was mapped to FE_HAS_SIGNAL, which is wrong. PLL
lock has nothing to do with received signal. In real life that flag
is always set.

Instead, let's enforce a limit for the buffer. Considering that I2C
transfers are generally limited, and that devices used on USB has a
max data length of 64 bytes for the control URBs.
So, it seem safe to use 64 bytes as the hard limit for all those devices.
On most cases, the limit is a way lower than that, but this limit
is small enough to not affect the Kernel stack, and it is a no brain
limit, as using smaller ones would require to either carefully each
driver or to take a look on each datasheet.

Reading firmware status register to detect whether firmware is
running or not didn't work 100% reliably. That register was
likely set by firmware itself which means it could not contain
reasonable values until firmware is up and running. Usually it
just worked as some garbage value was returned accidentally but it
appears that in some cases returned garbage value was 0x00 which
was considered "firmware is up and running" by the driver and
firmware loading was skipped leaving device to non-working state.
Fix problem by removing unreliable check and let the driver keep
count whether firmware is loaded or not.
