The kbuild documentation clearly shows that the documents
there are written at different times: some use markdown,
some use their own peculiar logic to split sections.
Convert everything to ReST without affecting too much
the author's style and avoiding adding uneeded markups.
The conversion is actually:
- add blank lines and identation in order to identify paragraphs;
- fix tables markups;
- add some lists markups;
- mark literal blocks;
- adjust title markups.
At its new index.rst, let's add a :orphan: while this is not linked to
the main index.rst file, in order to avoid build warnings.

Add SPDX license identifiers to all Make/Kconfig files which:
- Have no license information of any form
These files fall under the project license, GPL v2 only. The resulting SPDX
license identifier is:
GPL-2.0-only

Since IWLWIFI doesn't depend on MAC80211 anymore, it needs to depend
on CFG80211, because it uses a few symbols from it. Add the
dependency on CFG80211 accordingly.
Additionally, make IWLWIFI_LEDS depend on IWLMVM or IWLDVM, since it
doesn't need mac80211 but must be used for these.

It's not the iwlwifi module that depends on mac80211, but iwlmvm and
iwldvm. To reflect this better, make MVM and DVM Kconfig options
depend on MAC80211 instead.

'default false' should be 'default n', though they happen to have the
same effect here, due to undefined symbols ('false' in this case)
evaluating to n in a tristate sense.
Remove the default instead of changing it. bool and tristate symbols
implicitly default to n.

Enabling the RTPM Kconfig option can be fairly risky.
Runtime PM must be validated against a specific platform
before it can be safely enabled. Hence, it makes no sense
for distros and other big OS vendors to enable it since
they ship code to various systems and unknown platform.
Make sure that this is hinted properly by making the
IWLWIFI_PCIE_RTPM Kconfig option depend on EXPERT.

This Kconfig option allows to load a firmware for
debugging with a different name. This mechanism has not
been used for a few years now and replacing the firmware
file works as well.
Kill this Kconfig option and all the code that goes with it.

Add an initial implementation of runtime power management (RTPM) for
PCI devices. With this patch, RTPM is only used when wifi is off
(i.e. the wifi interface is down). This implementation is behind a
new Kconfig flag, IWLWIFI_PCIE_RTPM.

The split of iwlwifi into DVM and MVM was a long time ago now,
so we can remove the "default IWLWIFI" that we had to keep all
existing .config files with working defaults during the split.
This is no longer necessary, practically nobody should now be
upgrading a .config that's older than the split.
