Variants of proc_create{,_data} that directly take a seq_file show
callback and drastically reduces the boilerplate code in the callers.
All trivial callers converted over.

The pointer h is already initialized to codeclist_top so the second
identical assignment is redundant and can be removed. Cleans up clang
warning:

Drop the FSF's postal address from the source code files that typically
contain mostly the license text. Of the 628 removed instances, 578 are
outdated.
