Fix race between write() and resume() due to improper locking that could
lead to writes being reordered.
Resume must be done atomically and susp_count be protected by the
write_lock in order to prevent racing with write(). This could otherwise
lead to writes being reordered if write() grabs the write_lock after
susp_count is decremented, but before the delayed urb is submitted.

By specifying NO_UNION_NORMAL the ACM driver does only use the first two
USB interfaces (modem data & control). The AT Port, Diagnostic and NMEA
interfaces are left to the USB serial driver.

If acm_submit_read_urbs() fails in acm_port_activate(), error handling
code calls usb_autopm_put_interface() while it is already called
before acm_submit_read_urbs(). The patch reorganizes error handling code
to avoid double decrement of USB interface's PM-usage counter.

When writing data we were:
lock
do some work
unlock
call function
lock
do some work
unlock
return
return
It turns out, that "function" was only ever called in the one place, so
instead of locking/unlocking for no good reason, just inline the
function and only grab the lock once.
This has sped up the pathological case of sending 1 byte packets to a
loop-back cdc-acm device from 49600 bytes per second to 50100 bytes a
second on my workstation. A tiny increase yes, but noticable, and now
the spinlock isn't the hottest thing on the perf graph anymore. Yes, we
are still waiting for the hardware for the most part, but getting rid of
a spinlock_irq_save() call for every packet is still a good thing.
And we end up deleting lines of code, always a win overall.
This was found by using a Teensy 3.0 device and the test program and
firmware located at:

Unregister tty device in disconnect as is required by the USB stack.
By deferring unregistration to when the last tty reference is dropped,
the parent interface device can get unregistered before the child
resulting in broken hotplug events being generated when the tty is
finally closed:

acm_probe() ignores errors in tty_port_register_device()
and leaves intfdata pointing to freed memory on alloc_fail7
error path. The patch fixes the both issues.

port_get, hangup, kref_put. Now the only thing needed is to call
tty_port_tty_hangup which does exactly that. And they can also decide
whether to consider CLOCAL or completely ignore that.

One exception is ifx6x60 where tty_wakeup was open-coded. We now call

The IMS PCU (Passenger Control Unit) device used custom protocol over serial
line, so it is presenting itself as CDC ACM device.
Now that we have proper in-kernel driver for it we need to black-list the

Now, we start converting tty buffer functions to actually use
tty_port. This will allow us to get rid of the need of tty in many
call sites. Only tty_port will needed and hence no more
tty_port_tty_get in those paths.
Now, the one where most of tty_port_tty_get gets removed:
tty_flip_buffer_push.
IOW we also closed all the races in drivers not using tty_port_tty_get
at all yet.
Also we move tty_flip_buffer_push declaration from include/linux/tty.h
to include/linux/tty_flip.h to all others while we are changing it
anyway.

Some devices (ex Nokia C7) simply don't respond at all when data is sent
to some of their USB interfaces. The data gets stuck in the TTYs queue
and sits there until close(2), which them blocks because closing_wait
defaults to 30 seconds (even though the fd is O_NONBLOCK). This is
rarely desired. Implement the standard mechanism to adjust closing_wait
and let applications handle it how they want to.

We also are trying hard to remove the old dbg() macro, but the final
bits of that removal will be coming in through the networking tree
before we can delete it for good.

If a device specifies zero endpoints in its interface descriptor,
the kernel oopses in acm_probe(). Even though that's clearly an
invalid descriptor, we should test wether we have all endpoints.
This is especially bad as this oops can be triggered by just
plugging a USB device in.

Currently we have no way to assign tty->port while performing tty
installation. There are two ways to provide the link tty_struct =>
tty_port. Either by calling tty_port_install from tty->ops->install or
tty_port_register_device called instead of tty_register_device when
the device is being set up after connected.
In this patch we modify most of the drivers to do the latter. When the
drivers use tty_register_device and we have tty_port already, we
switch to tty_port_register_device. So we have the tty_struct =>
tty_port link for free for those.

Currently CDC-ACM devices stay throttled when their TTY is closed while
throttled, stalling further communication attempts after the next open.
Unthrottling during open/activate got lost starting with kernel
3.0.0 and this patch reintroduces it.

Hub-initiated LPM is not good for USB communications devices. Comms
devices should be able to tell when their link can go into a lower power
state, because they know when an incoming transmission is finished.
Ideally, these devices would slam their links into a lower power state,
using the device-initiated LPM, after finishing the last packet of their
data transfer.
If we enable the idle timeouts for the parent hubs to enable
hub-initiated LPM, we will get a lot of useless LPM packets on the bus
as the devices reject LPM transitions when they're in the middle of
receiving data. Worse, some devices might blindly accept the
hub-initiated LPM and power down their radios while they're in the
middle of receiving a transmission.
The Intel Windows folks are disabling hub-initiated LPM for all USB
communications devices under a xHCI USB 3.0 host. In order to keep
the Linux behavior as close as possible to Windows, we need to do the
same in Linux.
Set the disable_hub_initiated_lpm flag for for all USB communications
drivers. I know there aren't currently any USB 3.0 devices that
implement these class specifications, but we should be ready if they do.

Here's the big USB merge for the 3.4-rc1 merge window.
Lots of gadget driver reworks here, driver updates, xhci changes, some
new drivers added, usb-serial core reworking to fix some bugs, and other
various minor things.
There are some patches touching arch code, but they have all been acked
by the various arch maintainers.

All num, magic and owner are set by alloc_tty_driver. No need to
re-set them on each allocation site.
pti driver sets something different to what it passes to
alloc_tty_driver. It is not a bug, since we don't use the lines
parameter in any way. Anyway this is fixed, and now we do the right
thing.

We had a user report that running setserial on /dev/ttyACM0 didn't work.
He pointed at an old patch by Oliver Neukum from 2008 that never went anywhere..

I made some minor changes to get it to apply again, and got the user to retest on 3.1,

The diff below is against 3.3rc. The only difference between this and
the version the user tested is the removal of the if (!ACM_READY) test
Havard removed ACM_READY in 99823f457d
I'm unclear if there's need for a different test in its place.

On some failures, the country_code field of an acm structure is freed
without freeing the acm structure itself. Elsewhere, operations including
memcpy and kfree are performed on the country_code field. The patch sets
the country_code field to NULL when it is freed, and likewise sets the
country_code_size field to 0.

The ACM_READY() macro doesn't seem to do anything useful, and it may
prevent tty_wait_until_sent() from working properly when called from
close.
Previously, acm_tty_chars_in_buffer() returned 0 whenever
acm->port.count was 0. This means close() could return before all the
data has actually been written.

Rework the locking and lifecycle management in the cdc-acm driver.
Instead of using a global mutex to prevent the 'acm' object from being
freed, use the tty_port kref to keep the device alive when either the
USB side or TTY side is still active.
This allows us to use the global mutex purely for protecting the
acm_table, while use acm->mutex to guard against disconnect during
TTY port activation and shutdown.
The USB-side kref is taken during port initialization in probe(), and
released at the end of disconnect(). The TTY-side kref is taken in
install() and released in cleanup(). On disconnect, tty_vhangup() is
called instead of tty_hangup() to ensure the TTY hangup processing is
completed before the USB device is taken down.
The TTY open and close handlers have been gutted and replaced with
tty_port_open() and tty_port_close() respectively. The driver-specific
code which used to be there was spread across install(), activate() and
shutdown().

There's a race between the USB disconnect handler and the TTY close
handler which may cause the acm object to be freed while it's still
being used. This may lead to things like

This is the simplest fix I could come up with. Holding on to open_mutex
while closing the TTY device prevents acm_disconnect() from freeing the
acm object between acm->port.count drops to 0 and the TTY side of the
cleanups are finalized.
