When using a virt_boundary_mask, as done for NVMe devices attached to
mpt3sas controllers, we require an unlimited max_segment_size as the virt
boundary merging code assumes that. But we also need to propagate that to
the DMA mapping layer to make dma-debug happy. The SCSI layer takes care
of that when using the per-host virt_boundary setting, but given that
mpt3sas only wants to set the virt_boundary for actual NVMe devices, we
can't rely on that. The DMA layer maximum segment is global to the HBA
however, so we have to set it explicitly. This patch assumes that mpt3sas
does not have a segment size limitation, which seems true based on the SGL
format, but will need to be verified.

Enable interrupt coalescing only on high iops queues.
In ioc config page 1, offset 0x14 (ProductSpecific field) is used to
determine interrupt coalescing enabled/disabled on per reply descriptor
post queue group(8) basis. If 31st bit is zero, then interrupt coalescing
is enabled for all reply descriptor post queues. If 31st bit is set to one,
then user can enable/disable interrupt coalescing on per reply descriptor
post queue group(8) basis. So to enable interrupt coalescing only on first
reply descriptor post queue group (i.e. on high iops queues), set bit 0 and
31.
This configuration should reset during driver unload or shutdown to the
default settings. For this, the driver takes copy of default ioc page 1 and
copies back the default or unmodified ioc page1 during unload and
shutdown. This means that on next driver load (e.g. if older version driver
is loaded by user), current modified changes on ioc page1 won't take
effect.

This code refactoring introduces function pointers.
Host uses Request Descriptors of different types for posting an entry onto
a request queue. Based on controller type and capabilities, host can also
use atomic descriptors other than normal descriptors. Using function
pointer will avoid if-else statements

During expander reset handling, the driver invokes kernel function
scsi_host_find_tag() to obtain outstanding requests associated with the
scsi host managed by the driver. Driver loops from tag value zero to hba
queue depth to obtain the outstanding scmds. But when blk-mq is enabled,
the block layer may return stale entry for one or more requests. This may
lead to kernel panic if the returned value is inaccessible or the memory
pointed by the returned value is reused.

Instead of calling scsi_host_find_tag() API for each and every smid (smid
is tag +1) from one to shost->can_queue, now driver will call this API (to
obtain the outstanding scmd) only for those smid's which are outstanding at
the driver level.
Driver will determine whether this smid is outstanding at driver level by
looking into it's corresponding MPI request frame, if its MPI request frame
is empty, then it means that this smid is free and does not need to call
scsi_host_find_tag() for it. By doing this, driver will invoke
scsi_host_find_tag() for only those tags which are outstanding at the
driver level.
Driver will check whether particular MPI request frame is empty or not by
looking into the "DevHandle" field. If this field is zero then it means
that this MPI request is empty. For active MPI request DevHandle must be
non-zero.
Also driver will memset the MPI request frame once the corresponding scmd
is processed (i.e. just before calling
scmd->done function).

Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page. Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

This is to fix SYNC CACHE and START STOP command failures with
DID_NO_CONNECT during driver unload.
In driver's IO submission patch (i.e. in driver's .queuecommand()) driver
won't allow any SCSI commands to the IOC when ioc->remove_host flag is set
and hence SYNC CACHE commands which are issued to the target drives (where
write cache is enabled) during driver unload time is failed with
DID_NO_CONNECT status.
Now modified the driver to allow SYNC CACHE and START STOP commands to IOC,
even when remove_host flag is set.

Call sas_remove_host() before removing the target devices in the driver's
.remove() callback function(i.e. during driver unload time). So that
driver can provide a way to allow SYNC CACHE, START STOP unit commands
etc. (which are issued from SML) to the target drives during driver unload
time.
Once sas_remove_host() is called before removing the target drives then
driver can just clean up the resources allocated for target devices and no
need to call sas_port_delete_phy(), sas_port_delete() API's as these API's
internally called from sas_remove_host().

When an ioctl is sent to FW, and if there is a controller reset issued
before ioctl gets completed, then in controller reset path all the pending
ioctl commands are terminated from "mpt3sas_ctl_reset_handler" function.
This will wake up the waiting ioctl commands in ioctl path and print
timeouts which are actually not timeouts.
Introduced "mpt3sas_base_check_cmd_timeout" function to check and print
whether command got timed out (or) terminated due to Host reset.

Linux driver when receives Broadcast Asynchronous Event Notification (BAEN)
from the controller firmware, checks all pending I/Os at the driver level
and issues query task, abort task TMs. This is done in the driver to handle
drives which are connected with multiple initiators and undergoing target
resets. In the BAEN handling code, the I/Os issued to NVMe drives are also
handled and query task and abort task TMs are issued, which are not
necessary as there is no multi-initiator and no BAEN concept with NVMe
drives. Hence when the driver checks for pending I/Os it skips NVMe drives
at this moment.

In ioctl exit path driver refers ioc_list to free memory associated with
diag buffers and event_log pointer used to save events by driver.
If ctl_exit() func is called after unregistering driver, then ioc_list will
be empty and hence driver will not be able to free the allocated memory
which in turn causes memory leak.
So call ctl_exit() function before unregistering mpt3sas driver.

1) Manufacturing Page 11 contains parameters to control internal
firmware behavior. Based on AddlFlags2 field FW/Driver behaviour can
be changed, (flag tm_custom_handling is used for this)
a) For PCIe device, protocol level reset should be used if flag
tm_custom_handling is 0. Since Abort Task Set, LUN reset and Target
reset will result in a protocol level reset. Drivers should issue
only one type of this reset, if that fails then it should escalate to
a controller reset (diag reset/OCR).
b) If the driver has control over the TM reset timeout value, then
driver should use the value exposed in PCIe Device Page 2 for pcie
device (field ControllerResetTO).

In function _scsih_add_device, for each device connected to an
enclosure, driver reads the enclosure page(To get details like enclosure
handle, enclosure logical ID, enclosure level etc.)
With this patch, instead of reading enclosure page everytime, driver
maintains a list for enclosure device(During enclosure add event,
enclosure device is added to the list and removed from the list on
delete events) and uses the enclosure page from the list.

Events were not processed during driver unload, hence unloading of
driver doesn't complete when drives are disconnected while unloading of
driver. So don't block events in ISR path, i,e., remove the flag
ioc->remove_host so that events are getting processed during driver
unload. Thus allowing driver unload to complete by processing drive
removal events during driver unload.

This is mostly updates of the usual drivers: arcmsr, qla2xx, lpfc,
ufs, mpt3sas, hisi_sas. In addition we have removed several really
old drivers: sym53c416, NCR53c406a, fdomain, fdomain_cs and removed
the old scsi_module.c initialization from all remaining drivers. Plus
an assortment of bug fixes, initialization errors and other minor
fixes.

The firmware event workqueue should not be marked as WQ_MEM_RECLAIM
as it's doesn't need to make forward progress under memory pressure.
In the current state it will result in a deadlock if the device had been
forcefully removed.

Sending I/O through 32 bit descriptors to Ventura series of controller
results in IO timeout on certain conditions. This error only occurs on
systems with high I/O activity.
Changes in this patch will prevent driver from using 32 bit descriptor
and use 64 bit Descriptors

This configures shost max sector to 128, single reply descriptor post

This patch finishes all outstanding SCSI IO commands (but not other commands,
e.g., task management) in the shutdown and unload paths.
It first waits for the commands to complete (this is done after setting
'ioc->remove_host = 1 ', which prevents new commands to be queued) then it
flushes commands that might still be running.
This avoids triggering error handling (e.g., abort command) for all commands
possibly completed by the adapter after interrupts disabled.

This patch adds checks for 'ioc->remove_host' in the SCSI error handlers, so
not to access pointers/resources potentially freed in the PCI shutdown/module
unload path. The error handlers may be invoked after shutdown/unload,
depending on other components.
This problem was observed with kexec on a system with a mpt3sas based adapter
and an infiniband adapter which takes long enough to shutdown:
The mpt3sas driver finished shutting down / disabled interrupt handling, thus
some commands have not finished and timed out.
Since the system was still running (waiting for the infiniband adapter to
shutdown), the scsi error handler for task abort of mpt3sas was invoked, and
hit an oops -- either in scsih_abort() because 'ioc->scsi_lookup' was NULL
without commit dbec4c9040 ("scsi: mpt3sas: lockless command submission"), or
later up in scsih_host_reset() (with or without that commit), because it
eventually called mpt3sas_base_get_iocstate().
After the above commit, the oops in scsih_abort() does not occur anymore
(_scsih_scsi_lookup_find_by_scmd() is no longer called), but that commit is
too big and out of the scope of linux-stable, where this patch might help, so
still go for the changes.
Also, this might help to prevent similar errors in the future, in case code
changes and possibly tries to access freed stuff.
Note the fix in scsih_host_reset() is still important anyway.

Performance improvement using block layer tag.
Curent driver gets scsiio tracker and free smid from link list and array
based tracking managed by driver. Accessing list in main io path is
performance pentaly because of protection using spinlock
"scsi_lookup_lock".
In this patch:
1. Driver removes all link list access from main io path and
use scmd->request->tag to get free smid.
2. Instead of holding 'struct scsiio_tracker' in its own pool
driver can embed it into the scsi command.
Driver provides cmd_size in scsi_host_template, so that struct
scsiio_tracker is preallocated by scsi mid layer for each scsi command.
