Rename functions that are used by multiple devices. New devices
have been added and the function names and driver name are no longer
general enough for the functionality they provide.

Mark reported that having asterisks on the end of directory names
confuses get_maintainer.pl when it encounters subdirectories, and that
my name does not appear when run on drivers/firmware/efi/libstub.

Emulex developers' email addresses are now "@avagotech" instead of
