Yuval Basson 4cf66d70b5 RDMA/qedr: SRQ's bug fixes

QP's with the same SRQ, working on different CQs and running in parallel
on different CPUs could lead to a race when maintaining the SRQ consumer
count, and leads to FW running out of SRQs. Update the consumer
atomically.  Make sure the wqe_prod is updated after the sge_prod due to
FW requirements.

Michal Kalderon 8a69220b65 RDMA/qedr: Fix synchronization methods and memory leaks in qedr
[ Upstream commit 82af6d19d8 ]

Re-design of the iWARP CM related objects reference counting and
synchronization methods, to ensure operations are synchronized correctly
and that memory allocated for "ep" is properly released. Also makes sure
QP memory is not released before ep is finished accessing it.

Where as the QP object is created/destroyed by external operations, the ep
is created/destroyed by internal operations and represents the tcp
connection associated with the QP.

QP destruction flow:
- needs to wait for ep establishment to complete (either successfully or
  with error)
- needs to wait for ep disconnect to be fully posted to avoid a race
  condition of disconnect being called after reset.
- both the operations above don't always happen, so we use atomic flags to
  indicate whether the qp destruction flow needs to wait for these
  completions or not, if the destroy is called before these operations
  began, the flows will check the flags and not execute them ( connect /
  disconnect).

We use completion structure for waiting for the completions mentioned
above.

The QP refcnt was modified to kref object.  The EP has a kref added to it
to handle additional worker thread accessing it.

Amrani, Ram efe63c220e RDMA/qedr: Parse VLAN ID correctly and ignore the value of zero
Rename vlan_id field name to vlan as it contains more than the vlan_id.
Mask out non vlan id fields from vlan tag of the QED LL2 RX GSI
vlan output. As it is expected to be vlan id only.
Ignore vlan_id with value of zero.

Direct Packet Mode support may be disabled, e.g, due to limited

Michal Kalderon b262a06e64 qed*: qede_roce.[ch] -> qede_rdma.[ch]
Once we have iWARP support, the qede portion of the qedr<->qede would
serve all the RDMA protocols - so rename the file to be appropriate
to its function.

While we're at it, we're also moving a couple of inclusions to it into
.h files and adding includes to make sure it contains all type
definitions it requires.

Ram Amrani e57bb6be5e RDMA/qedr: Add 64KB PAGE_SIZE support to user-space queues
Add 64KB PAGE_SIZE support to user-space CQ, SQ and RQ queues.
De-facto it means that code was added to translate 64KB
pages to smaller 4KB pages that the FW can handle. Otherwise,
the FW would wrap (or jump to the next page)  when reaching 4KB
while the user space library will continue on the same large page.

Note that MR code remains as is since the FW supports larger pages
for MRs.

Michal Kalderon 0518c12f1f qed*: LL2 callback operations
LL2 today is interrupt driven - when tx/rx completion arrives [or any
other indication], qed needs to operate on the connection and pass
the information to the protocol-driver [or internal qed consumer].
Since we have several flavors of ll2 employeed by the driver,
each handler needs to do an if-else to determine the right functionality
to use based on the connection type.

In order to make things more scalable [given that we're going to add
additional types of ll2 flavors] move the infrastrucutre into using
a callback-based approach - the callbacks would be provided as part
of the connection's initialization parameters.

Mintz, Yuval be086e7c53 qed*: Utilize Firmware 8.15.3.0
This patch advances the qed* drivers into using the newer firmware -
This solves several firmware bugs, mostly related [but not limited to]
various init/deinit issues in various offloaded protocols.

It also introduces a major 4-Cached SGE change in firmware, which can be
seen in the storage drivers' changes.

In addition, this firmware is required for supporting the new QL41xxx
series of adapters; While this patch doesn't add the actual support,
the firmware contains the necessary initialization & firmware logic to
operate such adapters [actual support would be added later on].

Ram Amrani f449c7a2d8 RDMA/qedr: Dispatch port active event from qedr_add
Relying on qede to trigger qedr on startup is problematic. When probing
both if qedr loads slowly then qede can assume qedr is missing and not
trigger it. This patch adds a triggering from qedr and protects against
a race via an atomic bit.

Ram Amrani 91bff997db RDMA/qedr: Remove CQ spinlock from CM completion handlers
There is only a single event queue that triggers the completion
events for the RDMA CM and it is being processed serially. This means
that inherently there can no parallelism of CQ completion handler
callbacks, hence the lock is redundant.
