Fix a long standing bug with per device locking.
Each device has an associated spinlock for synchronizing access to
hardware and state information with the ISR. A single hardware card has
one or more devices.
Bug: Non ISR code correctly acquires and releases the per device lock.
ISR incorrectly always acquires and releases the lock of the first device
on the card.
The decoupled and list based nature of the ISR and deferred processing
interaction allowed this to work in normal operation. Exceptional events
like an application forcing hardware shutdown, reset, or reconfiguration
while active can trigger the bug.
Fixed ISR to acquire and release the per device lock.
One exception is manipulation of the GPIO card resource which is global
and effectively owned by the first device of the card. Non-ISR access to
GPIO resource is changed to use lock of first device on card.

This patch for SGI Altix/IA64 eliminates interval long timer holdoffs in
cases where we don't start an interval timer before the expiration time.
This sometimes happens when a number of interval timers on the same shub
with the same interval run simultaneously.

When all VT's are in use, VT_OPENQRY casts -1 to unsigned char before
returning it to userspace as an int. VT255 is not the next available

On VT-d supporting platforms the GGTT is allocated in a stolen mem
section separate from graphcis stolen mem. The GMCH register contains
a bitfield specifying the size of that region. Docs suggest that this
region can only be used for GGTT and PPGTT. Hence ensure that the
PPGTT is disabled and use the complete area for the GGTT.
Unfortunately the graphics core on G33/Pineview can't cope with really
large GTTs and the BIOS usually enables the maximum of 512MB. So
don't bother with maximizing the GTT on these platforms.

This is needed because the next patch adjusts this register and everything

Clearing the HPET interrupt enable bit disables interrupt generation
but does not disable the timer, so the interrupt status bit will still
be set when the timer elapses. If another interrupt arrives before
the timer has been correctly programmed (due to some other device on
the same interrupt line, or CONFIG_DEBUG_SHIRQ), this results in an
extra unwanted interrupt event because the status bit is likely to be
set from comparator matches that happened before the device was opened.
Therefore, we have to ensure that the interrupt status bit is and
stays cleared until we actually program the timer.

When the initialization code in hpet finds a memory resource and does not
find an IRQ, it does not unmap the memory resource previously mapped.
There are buggy BIOSes which report resources exactly like this and what
is worse the memory region bases point to normal RAM. This normally would
not matter since the space is not touched. But when PAT is turned on,
ioremap causes the page to be uncached and sets this bit in page->flags.
Then when the page is about to be used by the allocator, it is reported
as:

In this particular case:
1) HPET returns 3ed00000 as memory region base, but it is not in
reserved ranges reported by the BIOS (excerpt):

2) there is no IRQ resource reported by HPET method. On the other
hand, the Intel HPET specs (1.0a) says (3.2.5.1):

[1] For case where Timer Block is configured to consume IRQ0/IRQ8 AND
Legacy 8254/Legacy RTC hardware still exists, the device objects
associated with 8254 & RTC devices should not report IRQ0/IRQ8 as
"consumed resources".
So in theory we should check whether if it is the case and use those
interrupts instead.
Anyway the address reported by the BIOS here is bogus, so non-presence
of IRQ doesn't mean the "optional" part in point 2).
Since I got no reply previously, fix this by simply unmapping the space
when IRQ is not found and memory region was mapped previously. It would
be probably more safe to walk the resources again and unmap appropriately
depending on type. But as we now use only ioremap for both 2 memory
resource types, it is not necessarily needed right now.

Now, rw_verify_area() checsk f_pos is negative or not. And if negative,
returns -EINVAL.
But, some special files as /dev/(k)mem and /proc/<pid>/mem etc.. has
negative offsets. And we can't do any access via read/write to the
file(device).
So introduce FMODE_UNSIGNED_OFFSET to allow negative file offsets.

Remove the call to iucv_unregister() and remove the goto label
as unregistering is the last step in the hvc_iucv initialization.

A notifier chain is called whenever the vt code modifies a terminal
content, except for one case which is when the modification comes
through writes to /dev/vcs* devices. Let's add the missing notifier
invocation at the end of vcs_write() for that case too.

The /dev/vcs* devices are used, amongst other things, by accessibility
applications such as BRLTTY to display the screen content onto refreshable
braille displays. Currently this is performed by constantly reading from
/dev/vcsa0 whether or not the screen content has changed. Given the
default braille refresh rate of 25 times per second, this easily qualifies
as the biggest source of wake-up events preventing laptops from entering
deeper power saving states.
To avoid this periodic polling, let's add support for select()/poll() and
SIGIO with the /dev/vcs* devices. The implemented semantic is to report
data availability whenever the corresponding vt has seen some update after
the last read() operation. The application still has to lseek() back
as usual in order to read() the new data.
Not to create unwanted overhead, the needed data structure is allocated
and the vt notification callback is registered only when the poll or
fasync method is invoked for the first time per file instance.

Dan Rosenberg noted that various drivers return the struct with uncleared
fields. Instead of spending forever trying to stomp all the drivers that
get it wrong (and every new driver) do the job in one place.
This first patch adds the needed operations and hooks them up, including
the needed USB midlayer and serial core plumbing.

Vasiliy found that pci_disable_device is not called on fail paths in
mxser_probe. Actually, it is called from nowhere in the driver.
There are three changes needed:
1) don't use pseudo-generic mxser_release_res. Let's use it only from
ISA paths from now on. All the pci stuff is moved to probe and
remove PCI-related functions.
2) reorder fail-paths in the probe function so that it makes sense and
we can call them from the sequential code naturally (the further we
are the earlier label we go to).
3) add pci_disable_device both to mxser_probe and mxser_remove.
There is a nit of adding CONFIG_PCI ifdef to mxser_remove. it is
because this driver supports ISA-only compilations and it would choke
up on the newly added calls now.

Ttyprintk is a pseudo TTY driver, which allows users to make printk
messages, via output to ttyprintk device. It is possible to store
"console" messages inline with kernel messages for better analyses of
the boot process, for example.

The ports are char devices; do not have seeking capabilities. Calling
nonseekable_open() from the fops_open() call and setting the llseek fops
pointer to no_llseek ensures an lseek() call from userspace returns
-ESPIPE.

A process can request for SIGIO on host connect / disconnect events
using the O_ASYNC file flag using fcntl().
If that's requested, and if the guest-side connection for the port is
open, any host-side open/close events for that port will raise a SIGIO.
The process can then use poll() within the signal handler to find out
which port triggered the signal.

Explain in a comment why there's no need to reference-count the portdev
struct: when a device is yanked out, we can't do anything more with it
anyway so just give up doing anything more with the data or the vqs and
exit cleanly.

When a port got hot-unplugged, when a port was open, any file operation
after the unplugging resulted in a crash. This is fixed by ref-counting
the port structure, and releasing it only when the file is closed.
This splits the unplug operation in two parts: first marks the port
as unavailable, removes all the buffers in the vqs and removes the port
from the per-device list of ports. The second stage, invoked when all
references drop to zero, releases the chardev and frees all other memory.

This moves to using cdev on the heap instead of it being embedded in the
ports struct. This helps individual refcounting and will allow us to
properly remove cdev structs after hot-unplugs and close operations.

To convert to using cdev as a pointer to avoid kref troubles, we have to
use a different method to get to a port from an inode than the current
container_of method.
Add find_port_by_devt() that looks up all portdevs and ports with those
portdevs to find the right port.

If the host is slow in reading data or doesn't read data at all,
blocking write calls not only blocked the program that called write()
but the entire guest itself.
To overcome this, let's not block till the host signals it has given
back the virtio ring element we passed it. Instead, send the buffer to
the host and return to userspace. This operation then becomes similar
to how non-blocking writes work, so let's use the existing code for this
path as well.
This code change also ensures blocking write calls do get blocked if
there's not enough room in the virtio ring as well as they don't return
-EAGAIN to userspace.

RAW_SETBIND and RAW_GETBIND 32bit versions are fscked in interesting ways.
1) fs/compat_ioctl.c has COMPATIBLE_IOCTL(RAW_SETBIND) followed by
HANDLE_IOCTL(RAW_SETBIND, raw_ioctl). The latter is ignored.
2) on amd64 (and itanic) the damn thing is broken - we have int + u64 + u64
and layouts on i386 and amd64 are _not_ the same. raw_ioctl() would
work there, but it's never called due to (1). As it is, i386 /sbin/raw
definitely doesn't work on amd64 boxen.
3) switching to raw_ioctl() as is would *not* work on e.g. sparc64 and ppc64,
which would be rather sad, seeing that normal userland there is 32bit.
The thing is, slapping __packed on the struct in question does not DTRT -
it eliminates *all* padding. The real solution is to use compat_u64.
4) of course, all that stuff has no business being outside of raw.c in the
first place - there should be ->compat_ioctl() for /dev/rawctl instead of
messing with compat_ioctl.c.

All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.
The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.
New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time. Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.
The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.
Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

All these files use the big kernel lock in a trivial
way to serialize their private file operations,
typically resulting from an earlier semi-automatic
pushdown from VFS.
None of these drivers appears to want to lock against
other code, and they all use the BKL as the top-level
lock in their file operations, meaning that there
is no lock-order inversion problem.
Consequently, we can remove the BKL completely,
replacing it with a per-file mutex in every case.
Using a scripted approach means we can avoid
typos.

Instead of win_req_t, drivers are now requested to fill out
struct pcmcia_device *p_dev->resource[2,3,4,5] for up to four iomem
ranges. After a call to pcmcia_request_window(), the windows found there
are reserved and may be used until pcmcia_release_window() is called.

In commit e517a5e970 the call to
map_page_into_agp() got removed from intel_i830_setup_flush(), but the
counterpart call from intel_i830_fini_flush() to unmap_page_from_agp()
was left in place.
Additionally, the page allocated here never gets its physical address
used for sending to hardware, so there's no need to allocate it with
GFP_DMA32. Nor is __GFP_ZERO really necessary, as the page is used
only to store data to force flushing of some internal processor state.

These devices don't do any writeback but their device inodes still can get
dirty so mark bdi appropriately so that bdi code does the right thing and files

The intel gtt fake agp driver is the only agp driver to use dma
address remapping. So it makes sense to fold this code back into the
only user (and thus reduce the reliance on the agp code).
This patch does the first step by initializing (and remapping) the
scratch page in a new function intel_gtt_setup_scratch_page.
Unfortunately intel_gtt_cleanup had to move to avoid a forward
declaration. The new scratch page is not yet used, though.
v2: Refactor out scratch page teardown. Suggested by Chris Wilson on
irc. This makes it clear what's going on and results in a nice
symmetry between setup and teardown.

A userspace could submit a buffer with 0 length to be written to the
host. Prevent such a situation.
This was not needed previously, but recent changes in the way write()
works exposed this condition to trigger a virtqueue event to the host,
causing a NULL buffer to be sent across.

I found this while working on a Linux agent for spice, the symptom I was
seeing was select blocking on the spice vdagent virtio serial port even
though there were messages queued up there.
virtio_console's port_fops_poll checks port->inbuf != NULL to determine
if read won't block. However if an application reads enough bytes from
inbuf through port_fops_read, to empty the current port->inbuf,
port->inbuf will be NULL even though there may be buffers left in the
virtqueue.
This causes poll() to block even though there is data to be read,
this patch fixes this by using will_read_block(port) instead of the
port->inbuf != NULL check.

The file names are somehow misleading as the code is not specific to
AMD K8 CPUs anymore. The files accomodate code for other AMD CPU
northbridges as well.
Same is true for the config option which is valid for AMD CPU
northbridges in general and not specific to K8.

So far we only provide num_k8_northbridges. This is required in
different areas (e.g. L3 cache index disable, GART). But not all AMD
CPUs provide a GART. Thus it is useful to split off the GART handling
from the generic caching of AMD northbridge misc devices.

Some applications try to seek on tape devices
and fail if they return an error. Since we
want to change the default llseek action to
no_llseek, viotape needs to be changed to use

hpet uses the big kernel lock in its ioctl and open
functions. Replace this with a private mutex to be
sure. Since we're already touching the ioctl function,
add the compat_ioctl version as well -- all commands
except HPET_INFO are compatible and that one is easy
to add.

All these files use the big kernel lock in a trivial
way to serialize their private file operations,
typically resulting from an earlier semi-automatic
pushdown from VFS.
None of these drivers appears to want to lock against
other code, and they all use the BKL as the top-level
lock in their file operations, meaning that there
is no lock-order inversion problem.
Consequently, we can remove the BKL completely,
replacing it with a per-file mutex in every case.
Using a scripted approach means we can avoid
typos.

Several devices use a high number of bits for scancodes. One important
group is the Remote Controllers. Some new protocols like RC-6 define a
scancode space of 64 bits.
The current EVIO[CS]GKEYCODE ioctls allow replace the scancode/keycode
translation tables, but it is limited to up to 32 bits for scancode.
Also, if userspace wants to clean the existing table, replacing it by
a new one, it needs to run a loop calling the ioctls over the entire
sparse scancode space.
To solve those problems, this patch extends the ioctls to allow drivers
handle scancodes up to 32 bytes long (the length could be extended in
the future should such need arise) and allow userspace to query and set
scancode to keycode mappings not only by scancode but also by index.
Compatibility code were also added to handle the old format of
EVIO[CS]GKEYCODE ioctls.
Folded fixes by:
- Dan Carpenter: locking fixes for the original implementation
- Jarod Wilson: fix crash when setting keycode and wiring up get/set
handlers in original implementation.
- Dmitry Torokhov: rework to consolidate old and new scancode handling,
provide options to act either by index or scancode.

This way create_gatt_table become dummy glue functions for the fake
agp driver - rename them accordingly (and kill the now unnecessary
i9xx copy).
With this change, the gtt initialization code is almost independant
from the agp stuff. Two things are still missing:
- the scratch page is created by the generic agp code.
- filling the whole gtt with scratch_page ptes is not yet consolidated -
this needs abstracted pte handling, first.

The only difference between i915 and i965 was the calculation of the
gtt address. So merge these two paths into one. Otherwise the same

In commit f1befe71 Chris Wilson added some code to clear the full gtt
on g33/pineview instead of just the mappable part. The code looks like
it was copy-pasted from agp/intel-gtt.c, at least an identical piece
of code is still there (in intel_i830_init_gtt_entries). This lead to
a regression in 2.6.35 which was supposedly fixed in commit e7b96f28
Now this commit makes absolutely no sense to me. It seems to be
slightly confused about chipset generations - it references docs for
4th gen but the regression concerns 3rd gen g33. Luckily the the g33
gmch docs are available with the GMCH Graphics Control pci config
register definitions. The other (bigger problem) is that the new
check in there uses the i830 stolen mem bits (.5M, 1M or 8M of stolen
mem). They are different since the i855GM.
The most likely case is that it hits the 512M fallback, which was
probably the right thing for the boxes this was tested on.
So the original approach by Chris Wilson seems to be wrong and the
current code is definitely wrong. There is a third approach by Jesse
Barnes from his RFC patch "Who wants a bigger GTT mapping range?"
where he simply shoves g33 in the same clause like later chipset
generations.
I've asked him and Jesse confirmed that this should work. So implement
it.

agp/intel_gtt.c and drm/i915/i915_dma.c don't calculate this the same
way: The intel-gtt code seems to use the actual gtt size, the drm
module just the mappable. Go with the logic from the drm module because
that's the more conservative choice.
But conserve the original code in intel_gtt_total_size for later use.

This implementation is stolen from drm/i915, but is equivalent to
the code sprinkled over intel-gtt.c in the various fetch_size functions.
It's not yet used anywhere, though.
Also introduce intel_gtt_init which only calls intel_gtt_stolen_entries.
Over the course of the next patches, this will grow untill it contains
the complete init sequence starting from the call to gtt_mappable_entries.

Now that the disentangling is complete, stop including intel-gtt.c
from intel-agp.c.
The linux build system _really_ doesn't allow .c source files with the
same name as the module. It fails with the following message when trying
to build such a bugger:
make[3]: Circular drivers/char/agp/intel-agp.o <- drivers/char/agp/intel-agp.o dependency dropped.
Instead of renameing intel-agp.c I've simply created a new module out
of intel-gtt.c. Renaming intel-agp.ko to something else is not an option
for it will surely kill someones boot process.
This also paves the way to use the gtt code without loading the agp
driver.

This just splits the device list into two and moves the gtt related stuff
to intel-gtt.c. The two new devices lists also lose the not longer needed
fields. There where only about 5 cases anyway with both a gmch and a
possible agp port, so the duplication of entries is rather small.
Additionally kill 2 out of the three Ironlake mobile entries that
only differed in host bridge pci id.

Sandybridge GTT has new cache control bits in PTE, which controls
graphics page cache in LLC or LLC/MLC, so we need to extend the mask
function to respect the new bits.
And set cache control to always LLC only by default on Gen6.

Make sure we always detect when we fail to correctly allocate the Isoch
Flush Page and print an error to warn the user about the likely memory
corruption that will result in invalid rendering or worse.

Current code tramples over bit F3x90[6] which can be used to
disable GART table walk probes. However, this bit should be set
for performance reasons (speed up GART table walks). We are
allowed to do that since we put GART tables in UC memory later
anyway. Make it so.

I found a bug "by chance" in drivers/char/tty_io.c
I mean "by chance" because I was just reading the code of the
tty_find_polling_driver() to make a new tty_find_by_name() function.
In tty_find_polling_driver() the driver actually test "tty_line <=
p->num" while num refers to the number of struct tty_struct pointers
allocated for the p->ttys (p is a tty_driver), and tty_line is scanned
in a tty name, which can be for example ttyS2. Then tty_line equals 2.
And if p->num is 2, we have only p->ttys[0] and p->ttys[1], but no
p->ttys[2].
This is actually unharmful, for tty_find_polling_driver() is used only
in drivers/serial/kgdboc.c, and there's a test over there to find a
console with a matching index, which will never happen.
This is still a bug anyway.

After 02f0777a0d "vc_origin" is no
longer reset to the screen buffer before calling the con_init() hook
of the new console driver.
If the old driver wasn't using a fixed scanout buffer (e.g. the case
of vgacon) "vc_origin" may be a pointer to a VRAM location, and its
contents aren't guaranteed to be preserved after calling con_deinit()
on the old driver and con_init() on the new driver, i.e. the
subsequent console resize may fill the framebuffer with garbage.
It can be reproduced in the transition from vgacon to the nouveau
framebuffer driver: in that case the legacy VGA aperture "vc_origin"
points to becomes unreadable after fbcon_init().
This patch reverts the mentioned commit. To avoid the problem it
intended to fix, stop using "vc_scr_end" in vc_do_resize() to
calculate how many rows we have to copy (actually the code looks
simpler this way without the help of "vc_scr_end").

The KIOCSOUND and KDMKTONE ioctls are based on the CLOCK_TICK_RATE,
which is architecture and sometimes configuration specific.
In practice, most user applications assume that it is actually defined
as the i8253 PIT base clock of 1193182 Hz, which is true on some
architectures but not on others.
This patch makes the vt code use the PIT frequency on all
architectures, which is much more well-defined. It will change the
behavior of user applications sending the beep ioctl on all
architectures that define CLOCK_TICK_RATE different from
PIT_TICK_RATE.
The original breakage was introduced in commit bcc8ca099 "Adapt
drivers/char/vt_ioctl.c to non-x86". Hopefully, reverting this change
will make the frequency correct in more cases than it will make it
incorrect.

tty code abuses fu_list, which causes a bug in remount,ro handling.
If a tty device node is opened on a filesystem, then the last link to the inode
removed, the filesystem will be allowed to be remounted readonly. This is
because fs_may_remount_ro does not find the 0 link tty inode on the file sb
list (because the tty code incorrectly removed it to use for its own purpose).
This can result in a filesystem with errors after it is marked "clean".
Taking idea from Christoph's initial patch, allocate a tty private struct
at file->private_data and put our required list fields in there, linking
file and tty. This makes tty nodes behave the same way as other device nodes
and avoid meddling with the vfs, and avoids this bug.
The error handling is not trivial in the tty code, so for this bugfix, I take
the simple approach of using __GFP_NOFAIL and don't worry about memory errors.
This is not a problem because our allocator doesn't fail small allocs as a rule
anyway. So proper error handling is left as an exercise for tty hackers.
[ Arguably filesystem's device inode would ideally be divorced from the
driver's pseudo inode when it is opened, but in practice it's not clear whether
that will ever be worth implementing. ]

This patch fixes missing includes from a number of .c files because
the code (wrongfully) depended on prom.h including them. The include
of linux/of_address.h was removed in microblaze prom.h in commit
"of/address: Clean up function declarations" (sha1 id 22ae782f8), but
not fixed in some callers. This patch fixes them up.

Commit b45cfba4e9 (vt,console,kdb:
implement atomic console enter/leave functions) introduced the ability
to atomically change the console mode with kernel mode setting but did
not preserve the state of the console_blanked variable.
The console_blanked variable must be restored when executing the
con_debug_leave() or further kernel mode set changes (such as using
chvt X) will fail to correctly set the state of console.

There are different types of a fifo which can not handled in C without a
lot of overhead. So i decided to write the API as a set of macros, which
is the only way to do a kind of template meta programming without C++.
This macros handles the different types of fifos in a transparent way.
There are a lot of benefits:
- Compile time handling of the different fifo types
- Better performance (a save put or get of an integer does only generate
9 assembly instructions on a x86)
- Type save
- Cleaner interface, the additional kfifo_..._rec() functions are gone
- Easier to use
- Less error prone
- Different types of fifos: it is now possible to define a int fifo or
any other type. See below for an example.
- Smaller footprint for none byte type fifos
- No need of creating a second hidden variable, like in the old DEFINE_KFIFO
The API was not changed.
There are now real in place fifos where the data space is a part of the
structure. The fifo needs now 20 byte plus the fifo space. Dynamic
assigned or allocated create a little bit more code.
Most of the macros code will be optimized away and simple generate a
function call. Only the really small one generates inline code.
Additionally you can now create fifos for any data type, not only the
"unsigned char" byte streamed fifos.
There is also a new kfifo_put and kfifo_get function, to handle a single
element in a fifo. This macros generates inline code, which is lit bit
larger but faster.
I know that this kind of macros are very sophisticated and not easy to
maintain. But i have all tested and it works as expected. I analyzed the
output of the compiler and for the x86 the code is as good as hand written
assembler code. For the byte stream fifo the generate code is exact the
same as with the current kfifo implementation. For all other types of
fifos the code is smaller before, because the interface is easier to use.
The main goal was to provide an API which is very intuitive, save and easy
to use. So linux will get now a powerful fifo API which provides all what
a developer needs. This will save in the future a lot of kernel space,
since there is no need to write an own implementation. Most of the device
driver developers need a fifo, and also deep kernel development will gain
benefit from this API.
Here are the results of the text section usage:

As you can see, kfifo_put/kfifo_get creates a little bit more code than
kfifo_in/kfifo_out, but it is much faster (the code is inline).
The code is complete hand crafted and optimized. The text section size is
as small as possible. You get all the fifo handling in only 3 kb. This
includes type safe fix size records, dynamic records and DMA handling.
This should be the final version. All requested features are implemented.
Note: Most features of this API doesn't have any users. All functions
which are not used in the next 9 months will be removed. So, please adapt
your drivers and other sources as soon as possible to the new API and post
it.

User of the kernel fifo should never bypass the API and directly access
the fifo structure. Otherwise it will be very hard to maintain the API.

This is one of the most interesting users of module parameters in the
tree, so weaning it off the old-style non-const module_param_call
scheme is a useful exercise.
I was confused by set_param_int/get_param_int (vs. the normal
param_set_int and param_get_int), so I renamed set_param_int to
set_param_timeout, and re-used param_get_int directly instead of
get_param_int. I also implemented param_check_wdog_ifnum and
param_check_timeout, so now the ifnum_to_use and timeout/pretimeout
parameters can just use plain module_param().

When the console has been redirected, a hangup of the tty
will cause tty_release to be called under the big tty_mutex,
which leads to a deadlock because hangup is also called
under the BTM.
This moves the BTM deeper into the tty_hangup function so
we can close the redirected tty without holding the BTM.
In case of pty, we now need to drop the BTM before
calling tty_vhangup.

The tty locking now follows the rules for mutexes, so
we can replace the BKL usage with a new subsystem
wide mutex.
Using a regular mutex here will change the behaviour
when blocked on the BTM from spinning to sleeping,
but that should not be visible to the user.
Using the mutex also means that all the BTM is now
covered by lockdep.

This changes all remaining users of tty_lock_nested
to be non-recursive, which lets us kill this function.
As a consequence, we won't need to keep the lock count
any more, which allows more simplifications later.

Some wait_until_sent versions require the big
tty mutex, others don't and some callers of
wait_until_sent already hold it while other don't.
That leads to recursive use of the BTM in these
functions, which we're trying to get rid of.
This turns all cleans up the locking there so
that the driver's wait_until_sent function
never takes the BTM itself if it is already
called with that lock held.

We need to release the BTM in paste_selection() when
sleeping in tty_ldisc_ref_wait to avoid deadlocks
with tty_ldisc_enable.
In tty_set_ldisc, we now always grab the BTM before
taking the ldisc_mutex in order to avoid AB-BA
deadlocks between the two.
tty_ldisc_halt potentially blocks on a workqueue
function that takes the BTM, so we must release
the BTM before calling it.

tty_mutex is never taken with the BTM held, except for
two corner cases that are worked around here.
We give up the BTM before calling tty_release() in the
error path of tty_open().
Similarly, we reorder the locking in ptmx_open()
to get tty_mutex before the BTM.

We can use the port mutex for this and also for the hangup path so removing
the problematic use of the hangup mutex in this driver. Fix up the locking
on the various port flags while we are at it.
Ultimately this driver needs to be using tty_port_ helpers which would sort
this out far better.

The riscom8 board uses lock_kernel to protect bits of the port setting
ioctl logic. We can use the port mutex for this as the logic is internal
and will also lock set versus open (a locking property that has been lost
somewhere along the way)

Jesse's initial patch commit said:
"At panic time (i.e. when oops_in_progress is set) we should try a bit
harder to update the screen and make sure output gets to the VT, since
some drivers are capable of flipping back to it.
So make sure we try to unblank and update the display if called from a
panic context."
I've enhanced this to add a flag to the vc that console layer can set to
indicate they want this behaviour to occur. This also adds support to
fbcon for that flag and adds an fb flag for drivers to indicate they want
to use the support. It enables this for KMS drivers.

These are the changes needed for the kernel to support
LINEMODE in the server.
There is a new bit in the termios local flag word, EXTPROC.
When this bit is set, several aspects of the terminal driver
are disabled. Input line editing, character echo, and mapping
of signals are all disabled. This allows the telnetd to turn
off these functions when in linemode, but still keep track of
what state the user wants the terminal to be in.
New ioctl:
TIOCSIG Generate a signal to processes in the
current process group of the pty.
There is a new mode for packet driver, the TIOCPKT_IOCTL bit.
When packet mode is turned on in the pty, and the EXTPROC bit
is set, then whenever the state of the pty is changed, the
next read on the master side of the pty will have the TIOCPKT_IOCTL
bit set. This allows the process on the server side of the pty
to know when the state of the terminal has changed; it can then
issue the appropriate ioctl to retrieve the new state.
Since the original BSD patches accompanied the source code for telnet
I've left that reference here, but obviously the feature is useful for
any remote terminal protocol, including ssh.
The corresponding feature has existed in the BSD tty driver since 1989.

kernel will die on some platform when switch from vga mode to framebuffer
mode. The reason of this bug is that bind_con_driver reset vc->vc_origin

misc_deregister() returns an error only when it attempts to unregister
the device that is not registered. This is the driver's bug.
Most of the drivers don't check the return value of misc_deregister().
(It is not bad thing because most of kernel *_unregister() API always
succeed and do not return value)
So it is better to indicate the error by WARN_ON() in misc_deregister().

Make /dev/console get initialised before any initialisation routine that
invokes modprobe because if modprobe fails, it's going to want to open
/dev/console, presumably to write an error message to.
The problem with that is that if the /dev/console driver is not yet
initialised, the chardev handler will call request_module() to invoke
modprobe, which will fail, because we never compile /dev/console as a
module.
This will lead to a modprobe loop, showing the following in the kernel
log:

The kernel console interface stores the number of lines it is
configured to use. The kdb debugger can greatly benefit by knowing how
many lines there are on the console for the pager functionality
without having the end user compile in the setting or have to
repeatedly change it at run time.

Some BIOSes will claim a large chunk of stolen space. Unless we
reclaim it, our aperture for remapping buffer objects will be
constrained. So clamp the stolen space to 32M and ignore the rest.

Adding the ignored stolen memory back into the general pool using the
memory hotplug code is left as an exercise for the reader.

A side-effect of being able to use custom page allocations with the
sg_table is that it cannot reap the partially constructed scatterlist if
fails to allocate a page. So we need to call sg_free_table() ourselves
if sg_alloc_table() fails.

When "onboard video memory" is set do "disabled" in BIOS on Asus P4P800-VM
board (i865G), kernel oopses with memory corruption:

This patch moves the declaration of of_get_address(), of_get_pci_address(),
and of_pci_address_to_resource() out of arch code and into the common
linux/of_address header file.
This patch also fixes some of the asm/prom.h ordering issues. It still
includes some header files that it ideally shouldn't be, but at least the
ordering is consistent now so that of_* overrides work.

space register I830_GMCH_CTRL is a mirror of GMCH Graphics
Control. The correct macro for isolating the aperture size bits is
therefore I830_GMCH_GMS_MASK along with the attendant changes to the
case statement.

Fix subsequent suspends by issuing tpm_continue_selftest during resume.
Otherwise, the tpm chip seems to be not fully initialized and will reject
the save state command during suspend, thus preventing the whole system
to suspend.

Both of_bus_type and of_platform_bus_type are just #define aliases
for the platform bus. This patch removes all references to them and
switches to the of_register_platform_driver()/of_unregister_platform_driver()
API for registering.
Subsequent patches will convert each user of of_register_platform_driver()
into plain platform_drivers without the of_platform_driver shim. At which
point the of_register_platform_driver()/of_unregister_platform_driver()
functions can be removed.

The use of "hvc_con_driver" as the name for a file-static "struct
console" with a ".setup" field pointing to an __init function causes
a modpost warning, since a non-initdata structure points to init code.
Using "hvc_console" as the name triggers the hacky "*_console"
workaround in modpost to silence the warning, and is the same thing
that most of the other console drivers already do.
I made the same change in hvsi.c since I happened to notice it was
likely to suffer from the same problem.

None of these changes fix any actual bugs, but are just various cleanups
that fell out along the way. In particular, some unused #defines and
includes are removed, PREFETCH_STRIDE is added (the default is right for
our shipping chips, but wrong for our next generation), our tile-specific
prefetching code is removed so the (identical) generic prefetching code
can be used instead, a comment is fixed to be proper GPL and not just a
"paste GPL here" token, a "//" comment is converted to "/* */", etc.

Fix a regression introduced by ae74e823cb ("ipmi: add parameter to limit
CPU usage in kipmid").
Some systems were seeing CPU usage go up dramatically with the recent
changes to try to reduce timer usage in the IPMI driver. This was traced
down to schedule_timeout_interruptible(1) being changed to
schedule_timeout_interruptbile(0). Revert that part of the change.

The ipmi code will never register a PCI or Open Firmware driver if a
hardcoded device is provided by the user by providing device addresses via
the module parameters. This can cause us to attempt to unregister a
driver that was never registered, resulting in an oops. Keep track of
registration in order to avoid this.
Fixes a post-2.6.34 regression.

This reverts commit 962400e8fd, which was
entirely bogus.
The code used to multiply the character offset by "vc->vc_cols", and
that's actually correct, because 'd' itself is an 'unsigned short'. So
the pointer arithmetic already takes the size of a VGA character into
account. Changing it to use vc_size_row (which is just "vc_cols"
shifted up to take the size of the character into account) ends up
multiplying with the VGA character size twice.
This got reported as bugs for various other subsystems, because what it
actually results in is writing the 16-bit vc_video_erase_char pattern
(usually 0x0720: 0x07 is the default attribute, 0x20 is ASCII space)
into some random other allocation.
So Markus ended up reporting this as a ext4 bug, while to Torsten Kaiser
it looked like a problem with KMS or libata. Jeff Chua saw it in
different places.
And finally - Justin Mattock had slab poisoning enabled, and saw it as a
slab poison overwritten. And bisected and reverted this to verify the
buggy commit.

This change is the core kernel support for TILEPro and TILE64 chips.
No driver support (except the console driver) is included yet.
This includes the relevant Linux headers in asm/; the low-level
low-level "Tile architecture" headers in arch/, which are
shared with the hypervisor, etc., and are build-system agnostic;
and the relevant hypervisor headers in hv/.

The code uses vc->vc_cols instead of vc->vc_size_row by mistake, it
will cause half of the region which is going to clear remain
uncleared.
The issue happens in background consoles, so it's hard to observe.

In gsm_dlci_data_kick() we call gsm_dlci_data_sweep() with the
"gsm->tx_lock" held so we can't lock it again inside
gsm_dlci_data_sweep(). I removed that lock from and added one to
gsmld_write_wakeup() instead. The sweep function is only called from
those two places.

When a program that has a virtio port opened and blocked for a write
operation, a port hot-unplug event will later led to a crash when
SIGTERM was sent to the program. Fix that.

This fixes a sporadic oops at boot on G5 Power Macs. The table_end
variable has the address of the last byte of the table. Adding on
PAGE_SIZE means we flush too much, and if the page after the table
is not mapped for any reason, the kernel will oops. Instead we add
on 1 because flush_dcache_range() interprets its second argument as
the first byte past the range to be flushed.
